While we rarely initiate a MOLST form in the hospital. since physicians do not find it user friendly for an acute setting, the ethics committee has noted that there are situations where it has been very useful.
We do include MOLST in all advance directive education efforts especially since we have nursing home patients who are admitted with one.

I suggest you contact Pat Bomba, who serves on the National POLST Paradigm Task Force as the representative from New York. Her email is‎ Patricia.Bomba@lifethc.com. She can provide you with more information about POLST use in New York hospitals.

In Oregon some of the hospitals convert the orders on the POLST form into their own inpatient orders. There is a workgroup looking at standardizing this process across health systems. When the patient is discharged, any inpatient orders should be used to update, revise, or void the pre-existing POLST form.

Suzanne, I serve as the Chair of the MOLST Statewide Implementation Team and MOLST development and implementation since inception. MOLST was designed as a common community-wide form and has been approved for use in all settings, including the community, by the New York State Department of Health. MOLST DOH-5003 is written in plain language, conforms with the Family Health Care Decisions Act and is widely used in hospitals, including ICUs, across the state. The legal requirements/clinical & decision standards for issuing DNAR and witholding/withdrawing life-sustaining treatment in NYS varies based on who makes the decision and where the decision is made.
