This is a timely question as you identify a common problem faced by many hospitals. I assume you are thinking of patients not referred for palliative care consultation. ADs are important for everyone, however patients who meet criteria for referral to palliative care would be a place to start. Incorporating a question about ADs in the nursing admission template is a common approach. Our hospital has developed an algorithm for 'what happens next' according to specific roles. I will email this to you in case it is helpful.
