After the palliative care dr. does his/her assessment and care plan, do the team members visit the patient individually or as a team?

Hi, I think having multiple people in the room is a nice model. This would include the MD along with the RN, SW, and/or chaplain. It can be comforting for the patient to see the whole team and great for members of the team to work together, literally. Of course, if an individual patient prefers separate visits or if the team is "dividing and conquering," separate visits may make sense.
