As all of you know, this issue (in addition too all the other issues of palliative care) is extremely draining. I would offer a word of caution. Kathy Foley delivered a talk at a Yale Bioethics conference in 08. It was very disconcerting to hear the lack of transparency of the Oregon Death With Dignity patient population. This would be key to any discussion with a patient regarding PAD or PAS. These practices need to be placed under valid "scientific" and, not just, political scrutiny.
