Post Hospice Inpatient Facility MD/NP documentation

I recently started working as a NP in our Hospice inpatient facility. I am new to Hospice as well. I am interested in receiving input on the types of documentation used by NP's to capture their rounds on the patients...ie SOAP notes, templates. I am having difficulty finding a source that details what is exactly required by the guidelines to be included in our notes. Thanks so much.

There are several good documentation tools available on the CAPC website. Go to the CAPC Home Page; click on Tools for Palliative Care Programs; click on Business, Clinical and Quality Management tools for Palliative Care Programs; Click on Standardized Consult, Progress Notes and Billing Cards. According to Anne Monroe, Director of the Palliative Care Program at Hospice of the Bluegrass, NP rounding on a hospice inpatient unit typically involves documentation using Evaluation and Management (E & M) codes. The tools above address E & M requirements.
