There are several good documentation tools available on the CAPC website. Go to the CAPC Home Page; click on Tools for Palliative Care Programs; click on Business, Clinical and Quality Management tools for Palliative Care Programs; Click on Standardized Consult, Progress Notes and Billing Cards.
According to Anne Monroe, Director of the Palliative Care Program at Hospice of the Bluegrass, NP rounding on a hospice inpatient unit typically involves documentation using Evaluation and Management (E & M) codes. The tools above address E & M requirements. For further elaboration on E & M coding, go the CMS Physician Center: www.cms.hhs.gov/center/physician.asp
