I am working on a Breast Cancer Project to improve the services our hospitals provide. One of the things we are doing is reviewing the information given to patients throughout their journey from signs and symptoms onwards.

Several patients have mentioned that they get an enormous amount of information and would prefer if information was given in in the form of a cancer folder/journal. The folder would include sections on such things as patient details, medical record, appointments, information on options and treatment, contacts, personal reflections and so on.

I have read about Cancer Journals being used in the UK and Australia but am having trouble getting hold of any copies.

Does anyone out there know of places that use cancer journals?
