As a follow-up from Dr. Weissman's CAPC audio conference on March 19, 2008 entitled, "Part 1: Developing the Inpatient Virtual Care Unit," I would like to share with you two resources that were referenced during his presentation:

1. "Trigger Criteria-Palliative Care Pathway Screening Assessment"
