That is such exciting news! There are so many possibilities. I am interested in how this looks as well and I have some ideas, but would need to know more about your structure and how your team works to service 5 SNF's. Our hospital is located in Billings, Montana.

Great news! I am a PC social worker in a hospital in Maine. I do have resources for you and would like to talk, rather than write. I am in the hospital generally on Tuesday, Wednesday, and Thursday until 3:00. Call 207-662-3142 or page 207-767-8556.
