We don't have a particular care plan for end-of-life care, but we do have an extensive set of palliative care algorithms for symptom management and they can be found under the "tools" section on this website:

If the order for these "Palliatice Care symptom relief algorithms" has been put into our Cerner system for a patient, nurses are able to enter any orders as needed (as long as they follow the algorithm precisely). That being said, implementing these algorithms safely requires extensive training and other factors that we could discuss if you are interested. My e-mail address is listed on the post.
