Post Order Set for Inpatient Hospice Care

Does anyone have a template for standarized orders for hospice patients being cared for in a hospital inpatient setting? Any help would be greatly appreciated! Thank you.

we have standard orders that we use when a patient is transferred from home to hospital setting. doctors can change them or not use them at all but most choose to use them. If you would like me to fax a copy please send me your fax #. Karen khagen@ kishhospital.com
