hi matt. CAPC is about to put out a toolkit that will contain policies and procedures on this topic. Stoppain.org from Beth Israel in NYC has a PCAD- Palliative Care for the Actively Dying care pathway with order sets.
The CAPC audioconference done by Christine Ritchie and Rodney Tucker from UAB focused specifically on policies and procedures for palliative care.
