I practice in Utah & we have been using the POLST form for 2 few years now. It is widely accepted in all SNFs, ALFs and hospitals. Our team completes a POLST form on all patients we consult on prior to discharge. We also find that is guides many discussions with patients and families regarding advance care planning / goals of care.

Would you share your hospital policies that guide the use of POLST? Are they separate from the DNR policies?
