The experts on this are Joanne Lynn and colleagues. They recently published a book on their numerous QI projects on palliative, end-of-life, and hospice care. The book is, "The Common Sense Guide to Improving Palliative Care" by Joanne Lynn, Ekta Chaudhry, Anne M. Wilkinson, published by Oxford University Press, ISBN 0195310411 . I haven't read it yet, but can't think of any better source on the subject than Joanne Lynn and her colleagues, so I am assuming it is great. Let me know what you think.
