Post Rapid Cycle Quality Improvement

Excellent Webinar! As a person trained in rather formal research methodology, I have seen "the perfect become the enemy of the good." We get so caught up in the need for a randomized trial, and don't have the time, money or resources, so we do nothing.... The nimbleness of the Rapid Cycle process is appealing in the context of real world, applied science. I have a few real world questions, though:

1) How readily is this being embraced, and at which institutions? I see academic articles about it, and as I recall, skilled nursing facilities tend to be where this process is most popular. What's your experience sense?

2) What are the most common obstacles to implementing Rapid Cycle Quality Improvement? In particular, what have been the issues that have caused "higher ups" to balk at a proposal to try this out?
