At my institution it does, and, politically it is best to be asked to consult. I have heard of a few institutions that have criteria for automatic consults, but am not aware of who they are and how well it works. You are seeing pts and making suggestions for symptom management and possibly assisting with making decisions which may change the plan of care. It is best that the attending is in agreement with the consult in the 1st place.

Our organization requires a physician order as well (you would not be able to bill for a "consult" if you do not have a physician order requesting your services).

At our hospital, JACHO stated it should be a core service and can be generated by anyone. but it pays to be considerate of the MD's goals in the case.

We require a physician order for the physician to see the patient. A nurse, SW, etc can request a nurse consult but we do not then see the patient. We review the chart and do the screening evaluation. Usually this generates a physician consult.
