I'm not aware of any palliative care facilities outside of the hospital. There are some nursing homes who propose that they provide skilled palliation under the skilled level of care. Palliative patients in the hospital are acute care patients and payment for the stay is provided through the standard 3rd party payors. Palliative patients in the hospital meet utilization criteria for the acute hospital stay. Palliative care is not a destination but a medical specialty service that gets involved in the acute hospitalized patients care in the same manner a cardiologist would as a consultant for patients with heart disease. Hospice programs at times open facilities for residential, respite and hospice general inpatient care but this is a very detailed process that is outlined by medicare. These facilities would be under the administration of the hospice program. Hospice patients by definition are palliative patients but apart from the hospice facilities I don't know of any model that supports starting a palliative care facility.
