Post Starting a New Program

I am a Nurse Practitioner working as a Supervisor in an outpatient BMT Progam. Our practice desparately needs a Palliative Care Program, however I am having a difficult time convincing our Physicians. What is the best way to approach or educate them regarding the basics, as well as the urgency or necessity of a program? I believe Palliative Care is vital to our program. Thank you!

Hi. I wonder what you are observing that leads you to recognize the need for palliative care? Documenting this may be a good start in building your argument for adding services. Pain and symptom management issues? Suffering? lack of EOL decision making? Family suffering or support needs? You can start by documenting observations. Maybe a QI project looking at pt/ family coping with BMT? Satisfaction with services? Survey re: needs family or pts identify? Is there any research in the lietrature you can cite to make your case? While I am sure you are correct that palliative care would add a lot to the BMT service, often what speaks to administration and influential clinicians is data, and evidence based recommendations. PS: don't forget indicators/ observations/ reports of staff stress/ moral distress!

Hi There-

I am shocked that BMT physicians would not be entirely on board! But at any rate, you seem to be doing the right thing.

I too am in the midst of starting a program, and fortunately I had a lot of support. But in general, for statistics, I found CAPC and NHPCO to be the best resources for broad, informational studies that clearly show the need for palliative care.

Additionally, if you look in the CAPC palliative care screening tools, those undergoing BMT are clearly some of the patients recommended for consults, so use that evidence to your advantage.
