I am currently looking into generating a palliative care consultation on all patients over a certain age who present to the ED with subsequent need for admission who would benefit from an end of life care discussion including POLST discussion and any palliative care elements that would benefit transition to inpatient care and clarification of treatment goals and code status etc. I was curious if anyone has any experience trying to establish this type of relationship with a hospital/care facility.

There may be some sites using age alone, however, in my experience, it has typcially been a combination of factors without age or admission alone. I would point you to two resources that we have created in the IPAL-EM portfolio to include "ED Palliative Care Screening Tool" http://ipal-live.capc.stackop.com/downloads/ipal-em-palliative-care-ed-screening-tool.pdf. as well as the resource - Grudzen CR, Stone SC, Morrison RS. The palliative care model for emergency department patients with advanced illness. J Palliat Med 2011;14:945-50.
