Post starting a palliative care program in a SNF

We are interested in starting a palliative care program in our skilled nursing facilities because we are unable to have our own hospice due to no CON available. I have been reading some of the other forums about reimbursement, and start ups. I saw CPT codes for billing this service 99304-99318, what is the actual reimbursement amount for this specialty service being provided, is it just a physician or NP visit charge or is it a set amount per day like RUG levels are?
