There is a section here on the forum called "Palliative Out-Patient Services." There is a nice discussion going on there with folks who are doing the work in the community. On the CAPC website in the Tools section there are a couple of documents that I think you may find helpful as you evaluate or start a PC outpatient program. One of them:
