The SNF is lucky to have you interested in building a palliative care program as there is a significant need for palliative care in this setting. There are resources specific to long term care including articles, papers, on the capc.org website.
Models of palliative care in nursing homes include consultative practices such as an APN or MD collaborating with SNF staff ( social workers, nurses, chaplains), or facility based APNs such as GNPs, incorporating palliative care concepts with their practice. Some facilities use a quality improvement approach and implement processes associated with palliative care outcomes. An example of this would be implementing family conferences/goals of care conversations on admission and with any significant change in functional status/medical condition, or implementing POLST for appropriate patients.
Education of staff is crucial. As you may know, there is a geriatric-ELNEC and educational resources for nursing assistants through HPNA.
It's likely others can add to this over view list with other strategies that have proved successful.
