large community based oncolgy practice working with payors to design oncology pathways based on diagnosis. would like to include palliative care into the pathways. any experience, thoughts, comments?

Hi--I'd suggest you review the palliative care clinical practice guidelines from NCCN as a starting point--that is an excellent resource guide.
