We do primarily outpatient palliative care. Does anyone involved in a palliative care service do a consent for service form with patients? We do not currently and are wondering if we should...outlining a pt's. rights, our role, etc. Thank you.

Hi,
We do not at UCSF. The services we provide fall within the routine clinical care arrangements that patients have for all their outpatient care, so your service probably doesn't need any paperwork separate from what your institution requires. I look forward to hearing from other programs.

Hi. There are particular rules that govern licensed home care, so depending upon the rules you are operating under, it may be required. You may want to check with your local licensing/administrative bodies about which rules apply. We do home based palliative care here, and it is governed by our home care agency license, and yes: we do have clients sign consents for services (service agreements), bill of rights, etc.
