Post Adding Palliative care consulting to hospice

I am an NP in California, trying to get some information on establishing an inpatient palliative care consulting team for inpatient care. We are currently a home care based hospice, billing only though Medicare Part A. We will not be staff of the hospital, rather consultants. How exactly do we (MD and NP) provied this extra service to our local hospital and begin billing Medicare Part B? Can we apply for Medicare Part B under our current Hospice company? Do we need to establish another entity with a separate Tax ID number in order to apply for Medicare Part B? Once we have the CMS-855B, does each provider have to fill out a separate CMS 855I and CMS 855R? And lastly, can we use our current billing department at the Hospice, or do those have to be separated as well? I know this is long and legal, but it is SOOOOOO hard to find answers. If you have references to articles or Medicare policies etc. I would appreciate that too. Thank you, Leslie
