We have a LTC facility as a unit in our hospital. Our physicians are credentialed by the facility and provide consults on the unit. Most commonly the consults are for goals of care discussion and transition to withdrawal of vents and supportive care. We work closely with the facility's IDT. Generally, they like to keep the patients on their unit through the end-of-life phase of care since they have developed a relationship with the family.
