Post Palliative care in the outpatient clinic setting

In the recent audio-conference (3/19) by the same name, Michael W. Rabow, MD listed under "Pearls" (as in of wisdom?) "Start small". Does anyone have an idea of what we are referring to? We ran out of time for questions. Is this relative to staffing and resources or availability in different sites?

Hi, Sorry we ran out of time for more questions. The "Start Small" idea was the advice to make small, incremental steps in launching a program, rather than biting off more than you can chew. Outpatient palliative care can be expensive, resource intense with staff, and can quickly generate a large census of patients who require ongoing care. Promising low and selling high is always good advice. Suggesting to many potential referrers that outpatient services are available, but then now being able to provide adequate service or service in a timely manner will lead to real disappointment and those potential referrers might not come back. So, starting with a small clinic (1/2 day per week, within a discrete patient population or referring doc population) makes sense for most programs. Best,
