Our inpatient consultation program will be starting soon. Does anyone have a psychosocial form their social worker uses for their initial assessment? Case managers at our hospital collect much of the information on the example from Central Baptist Hospital on the CAPC website. Any help/examples would be appreciated!

Some programs have standardized the psychosocial assessment process. We have developed a form that is used for our inpatient hospice and palliative care patients. It is completed by the MSW. I will send it to your individual e-mail.
