Some programs have standardized the psychosocial assessment process. We have developed a form that is used for our inpatient hospice and palliative care patients. It is completed by the MSW. I will send it to your individual e-mail.

I would like a copy of this form as well. We use the form that the hospital social workers use, which was already in place. I would really like to compare the information and see if we should continue to use this form, or develop a new one. Thanks!
