Our Palliative Care Program is developing a model for an LTAC. The CAPC calculator is not appropriate to make projections for this setting. Have you been able to develop assumptions to project capture rates, potential number of referrals, cost-savings, etc specifically for a palliative care consultative service housed in a LTAC? Any help you can provide would be appreciated. Furthermore, any other information you can provide about the the palliative care program you offer in Mount Carmel's LTAC would be helpful. Thank you.
