Designing a Program

Our PC in LTC initiative in the Memphis TN area is two-fold. (1) We have a pilot project going in a SNF and (2) we offer a PC program every 6 months to our LTC NP group.
The pilot project involves classes for the staff at a SNF (8 hours of class, broken into two 4-hour sessions)to explain the philosophy of PC in the LTC setting, identification of PC patients and care of those identified patients. We are ready to evaluate the first set of classes now and then we will offer this to another SNF. Eventually we would like the facilities to conduct their own courses by the "train the trainer" method.
The speakers for the NP association keep providers in LTC updated on the PC initiative and the PC philosophy.
Since I have a LTC practice already, I offer PC to my patients and families who are candidates.
We have various Hospice agencies available in the nursing homes where I practice, so I also offer this course of care if they so desire.
