Karen, I am in agreement with "Mary" in the implementation of a PC program in a SNF. I am the DON of a facility and am working on such a program. The "POLST" form will be a very important part of our program. If you are not familiar with POLST you can check it out at POLST.org. I would love to hear from anyone else that has a palliative care program in their buildings. Angie
