Hi Karen...it is most appropriate that a palliative care initiative can be implemented in a SNF. We have foure LTC's in southern OH with robust palliative care programs that collect outcomes around unwanted readmissions to hospitals with palliative care plan in place for "no transfer". We have an entire care process set up around how we identify, care for and communicate these resident wishes. Most care in the LTC is not so much palliative as it is rehabilitative (because that is how it is best reimbursed) so there is a great need to develop your care systems so that the resident's needs are met as they progress in medical or natural functional decline.
