Post Inpatient Palliative Care in a Rural Hospital

We are investigating the feasibility of starting a formal palliative care program in our rural hospital. When I say rural, I mean a 24 bed Critical Access Hospital. Does anyone have any experience starting such a program, or resources to share regarding a rural setting specifically?
