We are an acute care hospital and in the infancy of starting a program. I am trying to find a good way of tracking patients if they are re-admitted and have not been followed by an agency such as Hospice or Pallative care. We use meditech as our computer program.

Ideally, you'd be able to use clinical computer systems and the admissions department to receive automated or nearly automated alerts when a patient previously seen by inpatient palliative care service is re-admitted to the hospital. I think a lot of us are struggling to make that vision a reality, and that it's not happening as quickly or as easily as we think it should. Other steps you could take in the meantime, while those depts are working on that, are to: 1) Educate and enlist the Emergency Dept to notify you of any returning patients entering through the ED. 2) Make sure that you are providing excellent communication to your referring physicians and your pts' primary care providers regarding your service, and include with that a request that they notify you promptly of the next admission for those patients so that you may assist in their care as quickly as possible. 3) Make sure you receive a daily printout of new admissions and have your lead clinician scan it for your PC patients -- they will be able to recognize a surprising number of names of those they have worked with. These and other steps like these are, of course, just stop-gaps along the way to the ideal solution. I also am a palliative care nurse in an acute care setting and just started a program. We have the EPIC computer system and there is a "FYI" tab that was built into the heading at the top of the page where name and other info appears. I have a special "FYI" security clearance on my login. When I am consulted on a pt, I click on the FYI tab and a screen comes up and I choose from the options, which for me is Palliative Care, and when I exit, the black FYI changes to Bold red FYI. Also, I have a consult list and an FYI list, so when I come in the next day there may be people on my FYI list that aren't on my consult list. I can then ask for a consult if necessary. Also, this FYI shows up to the admitting doctor as a best practice alert on admission, telling them they were P.C.

Hi! I am an RN in charge of the Inpatient Palliative Care Program at a community hospice. We also use Meditech. I keep an excel spreadsheet of all of our referrals. When they come back in, I can check new referrals against that spreadsheet list. It is also a small hospital and generally I remember the names.

I know that in Meditech you can click on the record and look at past admissions if you have any question if you have seen them before.
