Hi! I am an RN in charge of the Inpatient Palliative Care Program at a community hospice. We also use Meditech. I keep an excel spreadsheet of all of our referrals. When they come back in, I can check new referrals against that spreadsheet list. It is also a small hospital and generally I remember the names.

I know that in Meditech you can click on the record and look at past admissions if you have any question if you have seen them before.

We developed a Palliative Care Standard of Care and a Palliative Care Team Assessment for the interventions in Meditech here at PenBay.
