There is a shift in the field to looking at criterion based care and disease based indicators to identify the appropriate treatment plan and time for transition to a palliative plan of care. I don't have a policy on end of life care. Much of palliative care is navigating the grey and transitioning patients and the plan of care. This process would be based on criteria, goals of care, response to treatment etc. There are policies that could be put in place in terms of having advanced care planning and DNR discussions that could be helpful so that there aren't missed opportunities for facilitating and advocating the most appropriate plan of care.
