I'm with the Palliative Care Leadership Center in Lexington KY. Our program has an outpt. clinic, hospital consult service, and LTC consult service. We feel strongly that PC needs to cross the continuum of care. I would be happy to talk with you further.

Were you referring to a continuing care community, such as a life care facility? If so, I'm familiar with a successful physician -led program that serves a large CCRC.

I am from the Allina Health System in Minnesota. WE have inpatient consultation for patients and a community based program that sees patients in their home, NH, and Clinic setting. I would be happy to talk with you. Sandy 612-262-7063
