Post Palliative Care as a continuing care model

Is anyone aware of any Palliative Care programs that operates in a continuing care orgrnization. I work for an organization that offerers a full continuum os services ranging for skilled nursing, subacute care, home care, home hospice, and anchored by a rehaliilation hospital. However, since today's palliative care is about improving quailty of life and managing sysptoms at any stage of an illness, I feel it could be very useful to have a program that follows certain patients in different areas of the continuum.

I'm with the Palliative Care Leadership Center in Lexington KY. Our program has an outpt. clinic, hospital consult service, and LTC consult service. We feel strongly that PC needs to cross the continuum of care. I would be happy to talk with you further. Janet L. Were you referring to a continuing care community, such as a life care facility? If so, I'm familiar with a successful physician -led program that serves a large CCRC.

I am from the Allina Health System in Minnesota. WE have inpatient consultation for patients and a community based program that sees patients in their home, NH, and Clinic setting. I would be happy to talk with you. Sandy 612-262-7063
