Post Palliative Care as a continuing care model

Is anyone aware of any Palliative Care programs that operates in a continuing care orgrnization. I work for an organization that offerers a full continuum os services ranging for skilled nursing, subacute care, home care, home hospice, and anchored by a rehaliilation hospital. However, since today's palliative care is about improving quailty of life and managing sysptoms at any stage of an illness, I feel it could be very useful to have a program that follows certain patients in different areas of the continuum.
