Is there a good model of a palliative care unit that demonstrates excellent service delivery for both palliative care and hospice care?

There are such units. Two of our PCLC's - Palliative Care of the Bluegrass and Mount Carmel both have combined hospice and palliative care units. I might suggest contacting them for more information.
