Post Validated tool for initiation of palliative care

I was wondering if there are any tools which are validated for initiation of palliative care. Specifically for use in assessing patients who palliative care woud be appropriate for. I am familiar with the latest article with rcommendations but are there any tools???

Hello--tools to used as triggers for primary or speciality palliative care are pretty much home grown, and it seems that this approach works well; that is, to pick from the available tools/guidelines those triggers that seem most appropriate to your patients and to the needs of your referring clinicians. I strongly recommend that any such list be developed in collaboration with the key referring clinicians. To date there are no "validated" tools for use across multiple institutions.
