Data Collection and Measurement
Hi all!
I am working on a QI project for our Palliative Care Program, we are a consult service at a large teaching institution. The project is aiming towards obtaining consults earlier in the pt's stay. I am hoping to obtain LOS prior to PC consult data soon and am planning on implementing a PCST. I am leaning towards Caring Criteria which is a tool that gives an actual score and the score corresponds to whether a PC consult is needed for a pt. I am attempting to embed the Caring Criteria in the Medicine Service's admission order set so that all Medicine pt's are screened for a PC consult.
Has anyone implemented an admission screen at their institution? What is the best way to reach all Medicine interns/residents?
Would just like to hear some experiences, thanks!
