I will email you a copy of the phone survey we use at UCSF. We use the tool to gather information from both bereaved family members and for patients who are discharged alive. We phone a random sample of ten patients/families a month, which gives us an adequate sample without outstripping our resources. The survey is not formally validated, but we find the data to be quite useful.
