I am a psychologist in a community hospital in Toronto Canada. We are establishing an Outreach Palliaitve Care Program for palliative care referrals within the hospital for those patients who are not serviced through oncology.

We are looking for a triggering tool for the palliative care referral/assessment. I have found three on CAPC, but am in need of a tool with psychometric properties already documented/statistics from pilot study or program. Can anyone help with this?

I would really appreciate any assisitance that can be offered.

Hi Christine. I think if you are successful finding this type of tool we are all anxious to hear of it! As far as I am aware, I have not seen something like this --yet!
