This is an excellent question, and I suspect the lack of responses to date reflects the complexity of the issue. The ICU bundle by Judith Nelson and colleagues is a practical and setting specific (ICU) approach. Your experience of using the bundle across multiple settings would be of interest to many hospital systems. One of the biggest challenges would be implementation across varied clinical sites.
It's likely you are familiar with the recommended operational, customer, and clincal metrics as described in a series of articles in the Journal of Palliative Medicine and available as links on the capc website.
Other systems/states are focusing efforts on implementation of a standardized approach to advanced care planning and POLST for appropriate patients. In MN, we have worked to focus community efforts around the NQF 38 Preferred Processes, such as pain assessment and family conferences.
Another resource is the recent CAPC audioconference by Lynn Hill Spraegens and R. Sean Morrison MD ( November 9, 2010) that highlights the challenges for the field of palliative care and importance of working toward IOM goals.
