Post Utility of Mortality Chart Review

Our large hospital health care system is starting a new standardized mortality review process using retrospective chart review for all deceased patients. Is this a useful methodology for identifiying missed palliative care opportunities (i.e. hospice referral, goals of care discussion, symptom management)? Are there specific questions which would identify areas of excessive/non beneficial care? Can we really figure out what happened to the patient by reading the medical record?
