In terms of a patient satisfaction survey at our institution we worked with Press Ganey to develope what is termed a bereavement survey that is sent to the closest family member of all patients who die in the hospital. It has been a learning experience and now after 18 months of use we plan to dramatically modify it with the assistance of Press Ganey. This is available if you want to contact PG to inquire.
In general I would advise that surveys be simple, one page, and keeping in mind that you want to get the most information that would be of benefit to enhance your service. You can ask many questions and have tons of information and not know what to do with it. Also I would advise that you be consistent with other survey instruments used by your institution such as HCAPS so you may want to ask similar questions. This allows you to benchmark your experience with your institutions as a whole in certain areas, like maybe pain management. You would certainly want to do this in collaboration with your data/satisfaction department or experts at your institution.
