There is a company that has a data base product for palliative care. The website: http://www.lspragens.com/software.htm
You could contact them and if you like, they have a demo that they could send out. The three primary areas of palliative data collection are financial, clinical and satisfaction. I refer you to an article authored by Weissman, et al. for recommendations of data points to collect:
