I am currently working on my DNP and as my facility is building a PC program, I am doing my scholarly project on the referral piece of the program.I would be very interested in the methods used to educate staff on identifying referrals and whether you use the CAPC tool. Also, I am struggling with the measurement methods to ascertain a successful education program focused on referrals. Did your facility educate the physicians or just the nursing staff?
I think you could certainly use discharge dispositions as an outcomes measure especially in light of decreasing cost r/t LOS.

As far as education for physicians and nurses is concerned I am struggling with how to best accomplish this. There is still a lot of misunderstanding about what palliative care is so I have been presenting a basic "what is palliative care" presentation at medical staff meetings as well as nursing unit monthly meetings. This is helping some, but in order to focus on referrals I need to better educate the nursing staff. I am hoping to bring the ELNEC education to our organization. Also the screening tool we are currently utilizing is one from the online CAPC tools but also encorporates the Palliative Performance Scale.

If your goal is to increase referrals, remember there are four primary methods--
1) ensuring excellent care through consultation etiquette.
2) marketing your program features to the target audience
3) education about what your program can provide
4) systems change--changing the system of care in a manner that supports optimal use of palliative care.

All too often, staff think that education is the only method to get referrals--it is best to develop a strategy where all four methods are combined.
