Post Need help with assessment tools

I am very new to the palliative care scene and am working to build a CNS directed inpatient palliative care service. I would like to implement the Modififed Edmonton Symptom Assessment Scale (ESAS) and the Palliative Performance Scale version 2. I am having trouble determining whether these tools are public domain, or whether they require appropriate permissions. If anyone has already implemented these tools in their program, your guidance & suggestions would be much appreciated.
