Are you also familiar with the Seattle Heart Failure Model? It has a calculator as well.

Thank you for the additional model information. With a quick look of it I don't think it captures the patient's quality of life aspect, but could possibly be used for something further. I'll have to look into it a bit more, but I still think I have to use one of the other scales to focus on their quality of life.
