2 new tools, provided by the Symptom Management Service (SMS) at the University of California, San Francisco (UCSF), have been uploaded in the Tools Section of capc.org:

1. UCSF SMS Physician Clinical Data Form

2. UCSF SMS Patient Follow-up Satisfaction Survey
