and I'd recommend focusing on the pain and symptom surveys. Often, for acuity assessment, very brief surveys such as the ESAS are best and can be used frequently in re-assessments. Best, Mike

It became quite the buzz and we sold copies of it from Beloit Hospice where I was executive at the time. It went about through NHO (named that at the time) and WHO (Wisconsin).
I may be the only person with a copy somewhere. I've noticed that medical records are electronic now and I just imagined that my one-sheet acuity scale was no longer the way of things. But it's good.
So, all that to tell you that I'm hunting a copy and will share it with you when found. Are you still interested?
