Hi, I'm surprised there have been no replies to this post as I imagine assessing acuity is something that many of this Forum's readers do routinely in their clinical work. One place to start would be comprehensive symptom severity assessment. A key source of instruments is Joan Teno's Toolkit of Instruments to Measure End of Life. It's available here

and I'd recommend focusing on the pain and symptom surveys. Often, for acuity assessment, very brief surveys such as the ESAS are best and can be used frequently in re-assessments. Best, Mike
